HOLD HARMLESS AGREEMENT

 

We, the undersigned parent or legal guardinan of child/children's name(s):

agree to permit our child(ren) to participate in Biblical, scholastic, social, physical education, and other activities related to HERITAGE CREATIVE ARTS.  In consideration for the use of  GRACE UNITED METHODIST CHURCH  and the volunteer staff of HCA, we/I hereby waive and release any right or claim we/I may have against Grace United Methodist Church  and/or any HCA staff, and agree to hold them harmless from any and all claims, suits, losses, or related causes of action for damages, death, accidental or otherwise during or arising in any way from HCA classes, any HCA events on or off campus, and our children's use of Grace United Methodist Church .

Hold Harmless Agreement Explanation

The HERITAGE CREATIVE ARTS (HCA) Hold Harmless Agreement is a legal, required document to be signed by the child(ren)'s Parent or Legal Guardian prior to participation in any activity, event, program, etc. sponsored by Heritage Creative Arts.  Any questions regarding this policy should be directed to a member of the HCA Board.  The HCA Board consists of the Director, Assistant Director, Program Coordinator, Secretary, and Treasurer.
